You will find that this form does some automatic functions for you, for ex, adding the hours as you go (we don't want to 
go past the '99' hour mark, do we?).

Please note: Though the page should be functional, I don't have it in the final form. Some features of the original one
that I had at my disposal are not set up in this version. The calculations work fine, but the easy of entering some basic information such as your name
will take you an extra second or two. 
HINT: just put your cursor in the name {and member #} boxes and click a few times. Then you can just type. 

REMEMBER, THIS FORM WILL COMPUTE YOUR MONTHLY HOURS FOR YOU.
And, unlike some other form I have seen in the past few years, this one is SAVABLE to your computer.

Thus, you can print it when you are ready. The hours are cumulative. So you don't have to worry about going over 99.

PRINTING. I believe that when you use a standard size piece of paper (not 'legal' size), it will print just fine. Don't worry about the extra columns 
that show on the computer version of the form. They won't print.
